FCD Enhanced Elevation Certificate

The District has developed an interactive version of the FEMA Elevation Certificate form to assist users, primarily Engineers and Surveyors, in certifying elevation information necessary to ensure compliance with community floodplain management regulations, determine proper insurance premium rates, and to support a request to FEMA for a Letter of Map Amendment (LOMA) or Letter of Map Revision (LOMR). Using the Enhanced Elevation Certificate will result in faster form completion, fewer errors, and more accurate insurance rating. The enhanced version provides several user-friendly features:

  • Drop down text box — for Building Use and NFIP Community Name/Number. Click the tab to reveal the menu.
  • “Help” dialog box — press F1 on your keyboard to access instructions for data entry, helpful clarification, and other information.
  • Bottom text bar — at the bottom of the page the user will find abbreviated instructions and quick references.

For users not familiar with completing the form, you may wish to review the standard FEMA Elevation Certificate and complete Instructions. Additional guidance can be found in the FEMA Floodplain Management Bulletin available on FEMA's website. Click on “Elevation Certificate (FEMA 467- I).”

Note: All Elevation Certificates should include photographs per the form instructions. Copies of completed Elevation Certificates should be submitted to the Flood Control District and will be added to the website map.
