- Parent/Guardian must accompany minor if being vaccinated for consent

Pfizer Bivalent Booster (ages 6 months+) and Moderna Bivalent Booster (ages 6 months+) are administered at least two months after completion of the primary series or two months after the last Monovalent Booster.
